The Community Health Worker (CHW) provides outreach, health education, care coordination, and advocacy to Medi-Cal members and community members with complex needs. This is a highly field-based role, requiring strong relationship-building skills and the ability to navigate healthcare and social service systems effectively.

šŸ” Key Responsibilities

Community Outreach & Engagement

  • Conduct home visits, field outreach, and community-based engagement
  • Provide culturally responsive health education (mental health, substance use, chronic disease, wellness)
  • Lead or support community workshops, outreach events, and education campaigns
  • Conduct screenings to identify social determinants of health (SDOH) needs

Care Coordination & Navigation

  • Help individuals navigate healthcare, behavioral health, and social service systems
  • Connect clients to housing, benefits, food access, and other critical resources
  • Coordinate with providers, case managers, and community partners
  • Support engagement in treatment plans and long-term care goals

Advocacy & Relationship Building

  • Serve as a trusted advocate for clients and communities
  • Build authentic relationships grounded in cultural humility and lived experience
  • Empower individuals to take ownership of their health and well-being

Documentation & Compliance

  • Maintain accurate documentation in compliance with Medi-Cal and agency standards
  • Complete care plans, encounter notes, and screenings in a timely manner
  • Participate in team meetings, supervision, and ongoing training

 

āœ… Qualifications

Education & Experience:

  • High School Diploma or GED required
  • Associate’s or Bachelor’s degree in Public Health, Social Work, Psychology, or related field preferred
  • Minimum 2 years of experience in community outreach, health education, care coordination, or a similar role
  • CHW Certification Requirement (Medi-Cal):
  • Meets CHW requirements through either:
  • Completion of a CHW training program OR minimum 2,000 hours of CHW-related experience within the past 3 years (with certification obtained within 18 months if needed)

Preferred Experience:

  • Behavioral health, mental health, or substance use services
  • Lived experience that supports connection to communities served (highly valued)

Other Requirements:

  • Valid California Driver’s License, reliable vehicle, and current insurance
  • Ability to travel locally for field-based work (50–70%)
  • Ability to pass background clearances and meet health requirements
  • Strong relationship-building and communication skills
  • Knowledge of community resources and Medi-Cal/CalAIM programs
  • Ability to work with diverse populations with cultural humility
  • Basic computer skills and experience with documentation systems
  • Bilingual/bicultural skills strongly preferred
