Your Impact 

The Intake Social Worker (SW) conducts referral/ intake screening & assessment of youth referred for STRTP services. 

  • Screen calls for and schedule intake appointments.
  • Interview youth and/or placement worker, or CFT members to determine admission eligibility.
  • Follow up with all referral sources.
  • Maintain and/or establish new community-based organization relations for referrals.
  • Make recommendations to the Multi-Disciplinary Team for a youth’s admission.

 

Your Skills & Duties  

  • Conduct referral / intake screening & assessment of youth referred for STRTP services.
  • Screen calls for and schedule intake appointments.
  • Interview youth and/or placement worker, or CFT members to determine admission eligibility.
  • Follow up with all referral sources.
  • Maintain and/or establish new community-based organization relations for referrals.
  • Make recommendations to the Multi-Disciplinary Team for a youth’s admission.
  • Develop a thorough, written intake summary that includes reason(s) for referral, behavioral/emotional problems, dangerous propensities, current/past diagnosis, immediate needs/safety concerns, medication, medical history, etc.
  • Coordinate and schedule appointments for enrollment and day one activities.
  • Coordinate with placement committee to determine disposition. 
  • Complete and review intake packet with Multi-Disciplinary Team at weekly case review
  • Coordinate planning and seeking referrals for youth’s transfer or termination.
  • Maintain documentation on all applicants for STRTP in accordance with Federal, State, County, and agency guidelines.
  • Oversee the development and maintain an on-going list of community resources for referrals and linkages
  • Completes intake paperwork and scans/upload documents to EHR systems and/or WCM.
  • Provide reports on contract compliance activities and program outcomes
  • Attend seminars, conferences, and trainings relative to STRTP and trauma informed skills necessary for a position of leadership
  • Other duties as assigned.
  • Conduct and participate in Agency sponsored trainings and special events.  
  • Maintain appropriate professional boundaries with staff, clients and community resources.  
  • Participate in scheduled case review, individual and group supervision, and staff / program meetings.  
  • Maintain client and case confidentiality as per HIPAA, state and Agency standards.  
  • Secure and maintain adherence to program consents.  
  • Accurately complete administrative paperwork (time sheets, mileage, time off requests, expense reimbursements, weekly schedule, etc.) on time.  
  • Support and model Aspiranet’s values, represent the Agency in a professional manner, and abide by the Agency’s Professional Code of Ethics.  
  • Demonstrate sensitivity and responsiveness to cultural differences.  
  • Perform additional duties as assigned by Core Program Director/Manager/Supervisor.  

Education & Work Experience 

  • Masters Degree in an approved discipline, or with approval from the funder and/or regulatory body a Bachelors Degree in a related field and two years casework experience. For programs governed by Community Care Licensing, a formal exception must be granted by CCL. 

Licensing and Certifications 

  • Meets all state required conditions of employment as set forth by funder and regulatory body; i.e. Fingerprint and Child Abuse Index Clearance, TB/Health Physical, valid CDL and clean driving record. 
  • Meets all compliance requirements of federal, state, and country regulations by periodic sanction checks.

Compensation: $48000 - $54000 DOE
