Your Impact
The Permanency Social Worker provides comprehensive supportive services to adoptive and guardianship families focused on resources, education, advocacy, crisis intervention and hope. The Permanency Social Worker is responsible for information and referral, support groups, training and education, resource linkage, case management, and in-home supportive services.

Your Skills and Duties
- Bilingual (English/Spanish) required
- Provide individualized support and case management services for family, children and youth.
- Link clients to information and referral resources, adoption competent professionals, and services/programs for at risk children and families.
- Provide access to educational and resource materials, including agency adoption libraries.
- Provide crisis assessment/intervention and follow-up services.
- Facilitate support groups for children, youth and adults.
- Provide support to families, children, and youth after the legal finalization of an adoption to ensure families have ongoing long- term supports and services.
- Provide 1:1 and group education specific to parenting through a permanency lens.
- Provide support counseling and coaching to parents related to implementing trauma, attachment, and permanency informed techniques.
- Strengthen families and children in the early phases of the adoption process to increase protective factors that will minimize the likelihood of disruption/dissolution.
- Prepare and support adoptive and guardianship families as they move through the permanency process.
- Develop, implement and monitor client support plan and complete other case specific documentation as required.
- Identify and maintain community based relationships that promote effective service delivery to adoption and guardianship families and promote increased resource linkage to relevant pre and post adoptive supports.
- Develop resources for families that are permanency sensitive and adoption competent.
- Contribute to creation of quarterly newsletter.
- Support county partners that promote adoption of children within the foster care system, including promoting community based adoption promotion campaigns/activities, adoption trainings and forums.
- Coordinate with county partners on recognition of National Adoption Month and take part in associated activities.
- Understand and apply the following identified organizational practices: Strengthening Families 5 Protective Factors Framework, the Community Resiliency Model, (CRM), Life Space Crisis Intervention (LSCI) and Trust Based Relational Interventions (TBRI).
- Provide pre-placement and transition support services.
- Participate in the development and execution of community based trainings on permanency.
- Collaborate with other programs to develop and implement training for the larger agency on preparing adoptive parents for permanency.
- Work with local office to transition resource families to permanency, adoptive placement, and finalization and support post adoption needs.
- Take part in individual supervision and staff meetings; participate in the development of both short and long-term program and division planning.
- For PSSF funded programs, comply with data collection and outcome reporting required by the funding organization.
- Represent the agency in a professional manner, support and model Aspiranet’s vision and values, and abide by the agency’s Professional Code of Ethics.
- Maintain appropriate professional boundaries with staff, foster, adoptive, birth parents, children and community resources.
- Complete administrative paperwork (time sheets, mileage, time off requests, expense reimbursements, weekly schedule, etc.) on time.
- Travel to and from client homes to support the provision of in-home services.
- Commit to a flexible schedule that allows for some evening and occasional weekend work.
- Maintain client and case confidentiality per state, federal, licensing and agency standards.
- Demonstrate sensitivity and responsiveness to cultural differences.
- Perform additional duties as assigned or as required by County contract.
Education & Work Experience
- Master’s Degree in Social Work or similar discipline
- Adoption Experience preferred
- Proficient computer skills including: Microsoft Office: Excel; Word, Power Point; Outlook; SharePoint, Internet Explorer
- Meets all state required conditions of employment as set forth by Community Care Licensing for a Licensed Community Care facility; i.e. Fingerprint and Child Abuse Index clearance, TB/Health Physical, valid CDL and clean driving record.
Compensation: $66,560.00 - $68,360.00 Annually
