Working within an integrated community of care, the STRTP Social Worker provides case management services directly to children, youth, and families. The Social Worker will be responsible for conducting timely screenings and assessments, engaging the Child and Family Team (CFT) in the development and implementation of safety, needs, and service plans, and providing comprehensive case management services to support the well-being and successful transition of children and youth.

Your Skills and Duties

  • Conducts timely screenings and assessments, capturing identified strengths, needs, and safety concerns.
  • Works collaboratively to engage the Child and Family Team in the identification of strengths and the youth’s health, educational, religious, recreational, social, emotional, and psychological needs which inform the development and implementation of service plans.
  • Maintains responsibility for composing and updating youths' needs and services plan in coordination with the Integrated Care Coordinator (ICC) and Clinician. Collaborates closely with the ICC and Clinician to ensure that practices align consistently with the treatment plan. 
  • Administers case management services to address educational requirements, foster community connections, prepare for vocational endeavors, obtain essential documents, and provide additional necessary services for a smooth transition.
  • Serves as a liaison with collateral agencies and community partners to initiate and sustain relationships and services that bolster youth and their families in preparation for transition.
  • Provides case contact and support, including training, resources, advocacy, and interventions, as necessary to assist children, youth, care team members and their families in achieving service plan objectives.
  • Communicates all incidents, complaints, and grievances per program agency regulatory guidelines.
  • Applies, teaches and supervises to Aspiranet’s identified models of practice including the Aspiranet, Core Practice Model, Attachment Regulation and Competency (ARC), Youth Thrive / Strengthening Families Protective Factors Framework, Community Resiliency Model (CRM), Life Space Coaching and Crisis Intervention (LSCI), Professional Assault Crisis Training (ProAct), Motivational Interviewing (MI).
  • Adheres to all program, Agency, and State regulatory and policy requirements.
  • Completes timely and compliant documentation and meets productivity requirements.
  • Provides on-call emergency services in rotation by responding and, when necessary, mobilizing per Agency and program standards.
  • Adjusts caseload accordingly in order to support projects, initiatives, child and/or family training, and/or specific program needs.
  • Perform additional duties as assigned.

 

YOUR QUALIFICATIONS

  • Master’s Degree in an approved discipline, or with approval from the funder and/or regulatory body, a Bachelor’s Degree in a related field, and two years of casework experience. For programs governed by Community Care Licensing, a formal exception must be granted by CCL.
  • Meets all state-required conditions of employment as set forth by the funder and regulatory body, i.e. fingerprint and Child Abuse Index Clearance, TB/health physical, valid CDL, and clean driving record.
