Your Impact

Responsible for assisting clients and their families in gaining access to primary preventative and specialty health care services. Responsible for providing specific assistance with medication management, housing, counseling, support and advocacy. Additionally, responsible for engaging, educating, and offering support to individuals and their family in order to successfully connect them to culturally relevant health services including prevention, diagnosis, timely treatment, recovery management, and follow-up.

Your Skills & Duties

  • Provides emotional support to alleviate fears of and barriers to accessing quality health care.
  • Assists assigned clients to identify and progress toward achievement of their chosen goals.
  • Motivate and educate individuals/families/caregivers about the importance of preventative services, medical conditions and recovery strategies.
  • Advocates for clients in all areas of treatment and helps them apply for and receive services.
  • Empowers clients in meeting their goals.
  • Provides face-to-face contact with each client assigned.
  • Advocates for and assists clients and their families in accessing and receiving mental health treatment, medical care, housing, food and clothing as indicated.
  • Develops individual service plans for each client with their input whenever possible.
  • Meets regularly with clients at Aspiranet sites, jobsites, homes, board and care facilities, skilled nursing facilities, psychiatric facilities and other appropriate sites.
  • Arranges for or transports clients to and from medical appointments.
  • Provides prompt intervention in the event of a crisis both to stop the crisis and to notify, when indicated, persons and agencies necessary for the resolution of the crisis situation.
  • Consults with or meets regularly with the client’s family, care provider or other significant persons with client’s consent.
  • Coaches clients in developing age-appropriate social skills, money management, personal hygiene, and job readiness.
  • Assists in establishing client’s eligibility for Medi-Cal or other benefits and advocates for continuation of benefits when appropriate.
  • Interacts productively within the team in order to provide holistic mental health and case management services to an identified group of clients.
  • Provides “on-the-spot” counseling that is both helpful to the clients and consistent with the philosophy of the program.
  • Attends to the safety, health, and well being of clients.
  • Completes paperwork as assigned in a timely manner.
  • Adheres to and upholds the policies and procedures of Aspiranet.
  • Attends all team and agency staff meetings.

 Education & Work Experience

  • Bachelor's Degree in a social service/mental health related field and two years’ experience in that field
  • Meets all state required conditions of employment as set forth by Community Care Licensing for a licensed Community Care Facility; i.e. Fingerprint and Child Abuse Index Clearance, TB/Health Physical, valid California Driver’s License and clean | driving record (Motor Vehicle Report) & current vehicle insurance/registration.
  • Reliable means of transportation capable of passing vehicle safety inspection
  • Knowledge of psycho-social rehabilitation’s treatment and programming.
  • Ability to work and communicate with staff, clients, families, community agencies and professionals.
  • Ability to perform crisis intervention strategies.
  • Ability to work effectively under stress and conflict.
  • Ability to exercise appropriate judgment and decision-making.
  • Ability to be flexible and adaptable in any given situation.
  • Ability to work as a member of a team.
  • Ability to work under time deadlines and pressures.
  • Ability to work with minimal supervision.

Preferred

  • Bi-lingual in County Threshold language(s).
  • “Lived” experience is a plus.
